If you have made contact with us to enquire about the services we provide, we will need to collect and process some of your personal information in order to fulfil your enquiry. INFORMATION COLLECTED

When you make an enquiry you provide us with various information, such as your name, telephone

number and email address. You may also provide us with health data at this stage, which is

classed as a special category of personal data and is afforded additional protection under the regulations.

HOW YOUR DATA IS USED

We will use your data in order to respond to your enquiry and, if applicable (and with your consent) add you to a waiting list.

LAWFUL BASIS FOR PROCESSING

We process your data using the lawful basis of consent. As a recognised health care provider we are permitted to process your health-related data.

KEEPING YOUR DATA SAFE

All of the information relating to you will be held securely on the clinic’s computer system. This is

only accessed by those involved in your care and is backed up regularly. No information you

provide us is shared with a third party unless you have given us your explicit permission.

DATA RETENTION

Unless you become a client (at which point you will be issued with the appropriate privacy notice) your personal data will be removed after one month.

YOUR RIGHTS

• You have a right to request copies of the data we hold about you. You can request this verbally, in writing by email or by post to the clinic address

• You can request corrections of any incorrect data

• You can request the transfer of your data

• You can ask for your data to be deleted

DATA BREACH & COMPLAINTS

Where your personal data has been breached through loss or theft, and if affects your rights and freedoms as an individual, we will contact you immediately to explain the nature of the breach and how this is being managed.
